Western sleep science focuses primarily on sleep hygiene — the behavioral and environmental factors that influence sleep quality. TCM offers a complementary perspective: sleep disturbances are symptoms of underlying organ imbalances, and addressing those imbalances is the key to restoring truly restorative sleep.
The TCM Understanding of Sleep
In TCM, sleep is governed by the movement of Wei Qi (defensive energy) and the state of the Shen (spirit). During the day, Wei Qi circulates on the surface of the body, keeping us alert and active. At night, it moves inward to the Yin organs, allowing the Shen to settle and sleep to occur.
When this process is disrupted — by organ imbalances, emotional disturbances, or pathogenic factors — the Shen cannot settle, and sleep is disturbed.
Common TCM Patterns Behind Poor Sleep
Heart Blood Deficiency
Signs: Difficulty falling asleep, light sleep, vivid dreams, palpitations, poor memory, pale complexion, pale tongue, thin pulse.
Cause: Insufficient Blood to nourish the Heart and anchor the Shen.
Treatment: Nourish Heart Blood with herbs like Longan (Long Yan Rou), Ziziphus (Suan Zao Ren), and Angelica (Dang Gui).
Heart Yin Deficiency
Signs: Difficulty staying asleep, waking frequently, night sweats, palpitations, anxiety, restlessness, red tongue with little coating, rapid thin pulse.
Cause: Insufficient Yin to cool and anchor the Heart's Yang and Shen.
Treatment: Nourish Heart Yin and clear empty heat with Ophiopogon (Mai Men Dong), Rehmannia (Sheng Di Huang), and Ziziphus (Suan Zao Ren).
Liver Qi Stagnation
Signs: Difficulty falling asleep due to racing thoughts, waking between 1-3 AM, irritability, sighing, wiry pulse.
Cause: Stagnant Liver Qi disturbs the Shen and prevents relaxation.
Treatment: Move Liver Qi and calm the Shen with Bupleurum (Chai Hu), White Peony (Bai Shao), and Albizzia Bark (He Huan Pi).
Phlegm-Heat Disturbing the Heart
Signs: Difficulty falling asleep, restlessness, palpitations, feeling of heat, vivid or disturbing dreams, greasy yellow tongue coating, slippery rapid pulse.
Cause: Phlegm and heat combine to disturb the Heart and cloud the Shen.
Treatment: Clear heat, resolve phlegm, and calm the Shen with Coptis (Huang Lian), Pinellia (Ban Xia), and Poria (Fu Ling).
Kidney-Heart Disharmony
Signs: Difficulty falling asleep, waking frequently, palpitations, anxiety, low back ache, tinnitus, night sweats, red tongue, rapid pulse.
Cause: Kidney Yin is insufficient to nourish Heart Yin and cool Heart Fire, creating a disconnect between the Water (Kidney) and Fire (Heart) elements.
Treatment: Nourish Kidney Yin, clear Heart Fire, and reconnect Water and Fire with Coptis (Huang Lian), Cinnamon (Rou Gui), and Rehmannia (Di Huang).
Practical TCM Sleep Strategies
Evening routine: - Eat dinner before 7 PM — a full stomach disturbs sleep - Avoid screens after 9 PM — the blue light and mental stimulation disturb the Shen - Take a warm foot bath before bed — draws Qi and Blood downward, calming the mind - Practice gentle Qi Gong or meditation
Dietary support: - Longan fruit tea before bed (nourishes Heart Blood) - Warm milk with a pinch of nutmeg (calms the Shen) - Avoid caffeine after noon - Avoid alcohol — it disrupts sleep architecture
Acupressure: - Heart 7 (Shen Men): On the wrist crease, on the ulnar side — calms the Shen - Pericardium 6 (Nei Guan): Three finger-widths above the wrist crease — calms the Heart and mind - Kidney 1 (Yong Quan): Center of the sole of the foot — grounds energy downward
